Common adverse effects include bloating, flatulence, and fatty or oily stools. Oily spotting, increased fecal urgency or incontinence, and abdominal pain can also be experienced, especially when patients are noncompliant with a low-fat diet. Orlip is a modified bacterial drug that specifically inhibits intestinal lipase and can reduce fat and cholesterol absorption by approximately 30% in subjects eating a 30% fat diet. 540 Orlip irreversibly binds to the active site of the lipase, preventing the intraluminal deacylation of triglycerides and resulting in a 16 gram/day increase in fecal fat excretion. 541 Orlip does not inhibit other intestinal enzymes. It has minimal absorption and exerts no effect on systemic lipases. 542 , 543

Although there have been several open-label trials of Orlip in adolescents, only two randomized control trials (RCTs) have been published. 544 , 545 The side effects with Orlip are predictable from its mechanism of action on intestinal lipase. 534 Orlip appears to be well tolerated in adults, with the principal complaints being borborygmi, flatus, and abdominal cramps. The most troubling side effects are fecal incontinence, oily spotting, and flatus with discharge, which are highly aversive in the pediatric population. Orlip does not affect the pharmacokinetic properties of most other pharmaceutical agents. Absorption of vitamins A and E and β-carotene may be slightly reduced, and this may require vitamin therapy in a small number of patients. In one study, 546 vitamin D supplementation was required in 18% of subjects despite the prescription of a daily multivitamin containing vitamin D, although in the company-sponsored study, effects on vitamin levels were minor. 545 Orlip must be taken with each meal, which reduces its attractiveness for children, who are in school during lunchtime. Orlip is currently approved for the treatment of children as young as 12 years. An over-the-counter lower-dose preparation has obtained FDA approval and should be available soon.

Who should not take Orlip?

The following people should not take Orlip:

  • Pregnant or breastfeeding women.
  • People under the age of 18 years.
  • People who have a condition where food is not absorbed properly (a malabsorption syndrome).
  • People with cholestasis. This is a condition where bile does not flow properly from the liver to the duodenum.

Orlip (Xenical®)

Orlip acts as a reversible inhibitor of the gastric and pancreatic lipase and hence inhibits the hydrolysis of triglycerides into free fatty acids, with the result that dietary triglycerides are excreted undigested. This reduces fat absorption by about 30% at the usual therapeutic dose. Orlip is available in the European Union, United States and Australia as a non-prescription medication. Generic formulations are available in some countries. The recommended dose is 120 mg three times a day with meal. Higher doses do not give better clinical effects. It mainly acts locally in the gastrointestinal (GI) lumen and is predominantly excreted in faeces. Systemic absorption is minimal. The half-life of the systemically absorbed drug is about 1–2 h.

Orlip is one of the most studied pharmacological agents for its clinical efficacy on weight reduction. In the study on the use of Xenical in the Prevention of Diabetes in Obese Subjects (XENDOS study) , 3305 subjects were randomised to lifestyle changes plus Orlip or placebo. Compared to the placebo (lifestyle changes only) group, subjects in the Orlip group had significantly greater weight reduction (5.8 kg vs 3.0 kg, P . A more recent Cochrane review including 16 trials on long-term Orlip therapy showed a placebo-subtracted weight reduction of 2.9 kg (95% CI 2.5–3.2) and a significantly higher likelihood in achieving 5–10% weight loss in patients receiving treatment with Orlip. Subjects treated with Orlip also showed significant reductions in total, HDL and LDL cholesterol, blood pressure and incidence of diabetes mellitus . However, there has been no large clinical trial addressing cardiovascular end-points.

The most common side effect is GI symptoms including steatorrhoea, flatus with discharge, faecal urgency, increased defaecation and faecal incontinence. These GI effects may subside in 1–4 weeks. Users are recommended to avoid diet with high fat content, which could aggravate the GI side effects. As Orlip can reduce the absorption of fat-soluble vitamins (vitamins A, D, E, K and beta-carotene), patients taking Orlip are recommended to take multi-vitamin supplements at bedtime or at least 2 h before taking Orlip.

The use of Orlip is contraindicated in patients with chronic malabsorption syndrome, cholestasis and those with known hypersensitivity to this product. Orlip may reduce the plasma level of cyclosporin, and concurrent administration should be avoided. It can also impair the absorption of amiodarone.
