Sotret is a retinoid, which when administered in pharmacologic dosages of 0.5 to 1.0 mg/kg/day (see DOSAGE AND ADMINISTRATION), inhibits sebaceous gland function and keratinization. The exact mechanism of action of Sotret is unknown.
6. How and when to take it
It's important to take the capsules as instructed by your doctor.
As a general rule, the starting dose is 0.5mg per kilogram body weight per day (0.5 mg/kg/day). So if you weigh 60kg, your dose will usually start at 30mg a day.
Usually, you take Sotret capsules once or twice a day. Each capsule contains a dose of 10mg or 20mg of Sotret.
The dose of Sotret capsules varies from person to person, and depends on your weight.
Swallow the capsules whole with a drink of water. It's best to take them straight after a meal or snack to make sure they work properly.
Special precautions for pilots considering Sotret
Good night vision is important for airline pilots and those flying after dark. Night vision may be affected by Sotret. Pilots taking Sotret or considering a course of Sotret must report to their national aviation authority to discuss how this treatment affects their flying privileges.
In New Zealand, this is the Civil Aviation Authority of New Zealand. Civil aviation licence holders, including Air Traffic Controllers, have an obligation under section 27 C of the Civil Aviation Act to ground self and report to CAA in case of any change in medical condition that may affect flight safety. CAA considers the use of Sotret to be a change in medical condition.
Accutane (Sotret) must only be prescribed by prescribers who are registered and activated with the iPLEDGE program. Accutane (Sotret) must only be dispensed by a pharmacy registered and activated with iPLEDGE, and must only be dispensed to patients who are registered and meet all the requirements of iPLEDGE. Registered and activated pharmacies must receive Accutane (Sotret) only from wholesalers registered with iPLEDGE.

How does Sotret work?
In acne, Sotret:
- Reduces sebum production
- Shrinks the sebaceous glands
- Reduces follicular occlusion
- Inhibits the growth of bacteria
- Has anti- inflammatory properties.
Men and Sotret capsules
Men can safely take Sotret capsules if they and their partner are trying for a baby, or their partner is pregnant.
Sotret capsules don't seem to damage sperm. Only tiny amounts of Sotret get into semen, which is too little to harm the unborn baby.
