What is Pronicy (Periactin)?

Pronicy is an antihistamine that reduces the effects of natural chemical histamine in the body. Histamine can produce symptoms of sneezing, itching, watery eyes, and runny nose.

Pronicy is used to treat sneezing, runny nose, itching, red or watery eyes, and other symptoms of seasonal allergies (hay fever). Pronicy is also used to treat other conditions such as eczema or skin reactions to insect bites.

Pronicy is sometimes used to treat certain types of headaches, including migraines.

Pronicy may also be used for purposes not listed in this medication guide.

Geriatric Use

Clinical studies of Pronicy HCl tablets did not include sufficient numbers of subjects aged 65 and over to determine whether they respond differently from younger subjects. Other reported clinical experience has not identified differences in responses between elderly and younger patients. In general, dose selection for an elderly patient should be cautious, usually starting at the low end of the dosing range, reflecting the greater frequency of decreased hepatic, renal, or cardiac function, and of concomitant disease or other drug therapy (see WARNINGS, Activities Requiring Mental Alertness).

DOSAGE AND ADMINISTRATION

DOSAGE SHOULD BE INDIVIDUALIZED ACCORDING TO THE NEEDS AND THE RESPONSE OF THE PATIENT.

Each tablet contains 4 mg of Pronicy hydrochloride.

How should this medicine be used?

Pronicy comes as a tablet and a solution (liquid) to take by mouth. It is usually taken two or three times a day. Take Pronicy at around the same time(s) every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take Pronicy exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

If you are using the solution, do not use a household spoon to measure your dose. Use the measuring cup or spoon that came with the medication or use a spoon that is made especially for measuring medication.

Why is this medication prescribed?

It may also be used to relieve the itching of allergic skin conditions, and to treat hives, including hives caused by exposure to cold temperatures and by rubbing the skin. Pronicy is also sometimes used to treat allergic reactions in people who have received blood products as part of medical treatment and to treat life-threatening allergic reactions after the symptoms have been brought under control with other medications. Pronicy will help relieve symptoms but will not treat the cause of symptoms or speed recovery. Pronicy is in a class of medications called antihistamines. It works by blocking the action of histamine, a substance in the body that causes allergic symptoms.

Get emergency medical help if you have signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat.

Stop using Pronicy and call your doctor at once if you have:

  • a light-headed feeling, like you might pass out;
  • tremor, seizure (convulsions);
  • confusion, hallucinations;
  • little or no urination;
  • fast or pounding heartbeats;
  • easy bruising or bleeding;
  • ringing in your ears; or
  • pale or yellowed skin, dark colored urine, fever, weakness.

Common side effects may include:

  • drowsiness, dizziness;
  • dry mouth, nose, or throat;
  • constipation;
  • blurred vision; or
  • feeling restless or excited (especially in children).

Pronicy (Periactin)

Pronicy (Periactin), an antagonist at the 5-HT 2, histamine H1, L-calcium channels and muscarinic cholinergic receptors, is widely used in the preventive treatment of childhood migraine ( Barlow, 1984; Forsythe and Hockaday, 1988 ; Raskin, 1988b ). Curran and Lance (1964) found Pronicy to be more effective than placebo but less effective than methysergide. Pronicy (Periactin) is available as 4-mg tablets. The total dose ranges from 12 to 36 mg a day (given twice to three times a day or at bedtime). Common adverse events are sedation and weight gain; dry mouth, nausea, lightheadedness, ankle edema, aching legs, and diarrhea are less common. Pronicy may inhibit growth in children ( Smyth and Lazarus, 1974 ) and reverse the effects of SSRIs. Rao and colleagues (2000) reported that Pronicy (4 mg/day) was as effective as propranolol (80 mg/day) in reducing migraine frequency and severity. The combination of Pronicy and propranolol was more effective than monotherapy.

DESCRIPTION

Cyprohepdaine HCl, is an antihistaminic and antiserotonergic agent. Pronicy hydrochloride is a white to slightly yellowish crystalline solid, with a molecular weight of 350.89, which is soluble in water, freely soluble in methanol, sparingly soluble in ethanol, soluble in chloroform, and practically insoluble in ether. It is the sesquihydrate of 4-(5H-dibenzocyclohepten-5-ylidene)-1- methylpiperidine hydrochloride. The molecular formula of the anhydrous salt is C21H21N•HCl and the structural formula of the anhydrous salt is:

Please inform your doctor if you are taking any of the following medicines: anxiety or sleep medicines such as alprazolam (Xanax®), diazepam (Valium®), chlordiazepoxide (Librium®), temazepam (Restoril®), or triazolam (Halcion®); medications for depression such as amitriptyline (Elavil®), doxepin (Sinequan®), nortriptyline (Pamelor®), fluoxetine (Prozac®), sertraline (Zoloft®), or paroxetine (Paxil®); or any other medications that make you feel drowsy, sleepy, or relaxed.

Drugs other than those listed here may also interact with Pronicy. Talk to your doctor and pharmacist before taking any prescription or over-the-counter medicines. So too, do not take Pronicy if you have taken a monoamine oxidase inhibitor (MAOI) such as isocarboxazid (Marplan®), phenelzine (Nardil®), or tranylcypromine (Parnate®) in the last 14 days. A very dangerous drug interaction could occur, leading to serious side effects.

Furthermore, tell your doctor if you have glaucoma or increased pressure in the eye, a stomach ulcer, an enlarged prostate, bladder problems, or difficulty urinating, an overactive thyroid (hyperthyroidism), hypertension or any type of heart problems, or asthma. You may not be able to take Pronicy, or you may require a lower dose or special monitoring during treatment if you have any of the conditions listed above.

Pronicy is in the FDA pregnancy category B. This means that it is unlikely to harm an unborn baby. Do not take Pronicy without first talking to your doctor if you are pregnant.

Additionally, Pronicy passes into breast milk. Infants are especially sensitive to the effects of antihistamines, and serious side effects could occur in a nursing baby. Pronicy is not recommended if you are breast-feeding a baby. Do not take Pronicy without first talking to your doctor if you are nursing a baby.

If you are over 60 years of age, you may be more likely to experience side effects from Pronicy. You may require a lower dose of this medication.

Use cautiously when driving, operating machinery, or performing other hazardous activities. Pronicy may cause dizziness or drowsiness. If you experience dizziness or drowsiness, avoid these activities.

The consumption of alcohol may also increase these symptoms.

What should I do if I forget a dose?

Take the missed dose as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and take only your next regularly scheduled dose.

Do not take a double dose of this medication unless otherwise directed by your doctor.

Stop taking Pronicy and seek emergency medical attention if you experience:

sleepiness, fatigue, or dizziness headache dry mouth difficulty urinating or an enlarged prostate

Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome.

What storage conditions are needed for this medicine?

Always keep this medication in the container it came in, tightly closed, and out of reach of children. Store it at room temperature and away from excess heat and moisture (not in the bathroom).

Remember to throw away any medication that is outdated or no longer needed and, if required, talk to your pharmacist about the proper disposal of your medication.

What is Pronicy, and how does it work (mechanism of action)?

Pronicy (Periactin) is an oral antihistamine used for treating allergic reactions. It works by blocking the effects of histamine and is classified as a first generation antihistamine. Histamine is released by cells of the body during several types of allergic reactions and, to a lesser extent, during some viral infections, such as the common cold. When the histamine binds to receptors on other cells, it stimulates changes within the cells that lead to the release of chemicals that cause sneezing, itching, and increased production of mucus. Antihistamines compete with histamine for cell receptors and bind to the receptors without stimulating the cells. In addition, they prevent histamine from binding and stimulating the cells. Pronicy also blocks the action of acetylcholine (anticholinergic effect) and serotonin, two neurotransmitters that nerves and muscles use to communicate with one another, and it causes drowsiness.

How should I take Pronicy (Periactin)?

Follow all directions on your prescription label. Your doctor may occasionally change your dose to make sure you get the best results. Do not use this medicine in larger or smaller amounts or for longer than recommended.

Do not give this medicine to a child younger than 2 years old. You should not use antihistamine medication to make a child sleepy. Death can occur from the misuse of an antihistamine in very young children.

Measure liquid medicine with the dosing syringe provided, or with a special dose-measuring spoon or medicine cup. If you do not have a dose-measuring device, ask your pharmacist for one.

Tell your doctor if you have any changes in weight. Pronicy doses are based on weight (especially in children and teenagers), and any changes may affect the dose.

This medicine can cause you to have a false positive drug screening test. If you provide a urine sample for drug screening, tell the laboratory staff that you are taking Pronicy.

Store in a tightly-closed container at room temperature, away from moisture and heat.
