Iconal is a nonstero >N -(5-methyl-2-thiazolyl)-2 H -1,2-benzothiazine-3-carboxam >14 H 13 N 3 O 4 S 2 and it has the following structural formula.
- NSAIDs may cause an increased risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal. This risk may increase with duration of us e. Patients with cardiovascular disease or risk factors for cardiovascular disease may be at greater risk (see WARNINGS and Clinical Trials).
- Iconal is contraindicated for the treatment of peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery (see WARNINGS).
What is Iconal, and how does it work (mechanism of action)?
Iconal is in a class of drugs called nonsteroidal anti-inflammatory drugs (NSAIDs) that are used to treat pain and/or inflammation. Other members of this class include ibuprofen (Motrin), indomethacin (Indocin), nabumetone (Relafen) and several others. Prostaglandins are chemicals that contribute to inflammation especially within joints, and it is the inflammation that leads to the common symptoms of pain, tenderness, and swelling associated with arthritis. Iconal blocks the enzymes that make prostaglandins (cyclooxygenase 1 and 2) and reduces the levels of prostaglandins. As a result, inflammation and its accompanying symptoms are reduced. Iconal was approved for use in April 2000.
Antidepressants and anxiety drugs
Taking Iconal with certain antidepressant and anxiety medications raises your risk of bleeding. Examples of these drugs include:
- selective serotonin reuptake inhibitors, such as citalopram
- selective serotonin and norepinephrine reuptake inhibitors, such as venlafaxine

I have had both knees and my right hip replaced, from sport related wear. I'm 63 years old and have been using anti inflammatories since mid 30's. I won't name the actual ones I used, but the two I initially used caused issues, I was then prescribed Iconal and had no more problems I guess it depends on the individual, but i found they agreed with me. Now that I have had the joints replaced, I still use the anti inflammatory if I know I'm going to have a problem. like today when I cycled under a tree and nearly took my head off. Iconal and Kidney Function
There is also a danger that Iconal can impair the ability of the kidneys to function. Due to this, it is dangerous for patients who have kidney problems or who are at risk for kidney failure. Elderly patients, those with heart conditions and liver dysfunction, and patients receiving diuretics, ACE-inhibitors, or angiotensin II antagonists should also avoid Iconal. Additionally, some patients might develop serious skin ailments like toxic epidermal necrolysis, Stevens-Johnson syndrome, and exfoliative dermatitis. ,
Females and Males of Reproductive Potential
Based on the mechanism of action, the use of prostaglandin-mediated NSAIDs, including Iconal, may delay or prevent rupture of ovarian follicles, which has been associated with reversible infertility in some women. Published animal studies have shown that administration of prostaglandin synthesis inhibitors has the potential to disrupt prostaglandin-mediated follicular rupture required for ovulation. Small studies in women treated with NSAIDs have also shown a reversible delay in ovulation. Consider withdrawal of NSAIDs, including Iconal, in women who have difficulties conceiving or who are undergoing investigation of infertility.
Iconal for Dogs and Cats
Iconal can be prescribed for for dogs and cats to manage their pain and inflammation.
In addition to arthritis pain, Iconal may be prescribed to dogs for pain from surgery, dental procedures, cancer, or injuries.
Because of the increased risk of side effects in cats, Iconal has limited use in cats and is generally only prescribed as a single dose.
Side effects can include loss of appetite, vomiting, diarrhea, bleeding, and stomach ulcers.
Remember, keep this and all other medicines out of the reach of children, never share your medicines with others, and use Iconal only for the indication prescribed.

Generally speaking, the daily recommended dose of Iconal is 7.5 mg.
Your doctor may increase the dosage to 15 mg.
Iconal should be taken exactly as prescribed, at the lowest dose possible for your treatment and for the shortest time needed.
Take Iconal with a glass of water. Swallow the pill whole; don't chew it or let it melt/dissolve in your mouth.
Take Iconal with or without food. Taking it with food can help prevent an upset stomach.
Also, wait at least 30 minutes after taking Iconal before lying down to help prevent an upset stomach.
Prospective studies show that up to 7% of patients taking Iconal experience at least transient serum aminotransferase elevations. These may resolve even with drug continuation. Aminotransferase elevations >3 fold elevated occur in 1% of patients. Clinically apparent liver injury with jaundice from Iconal is rare and only individual case reports have been published. The latency to onset in reported cases was short (1-5 weeks) and both cholestatic and hepatocellular patterns of enzyme elevations were described. Immunoallergic features are usually not prominent and autoantibodies are rare, although a single case report of autoimmune hepatitis apparently triggered by Iconal therapy has been published. Recovery is typically rapid once Iconal is stopped. Iconal is rarely mentioned as an etiologic agent in large case series on drug induced liver injury and acute liver failure.
What Are NSAIDs and Iconal?
Nonsteroidal anti-inflammatory drugs have many other drugs under their umbrella: ibuprofen, nabumetone, and indomethacin, to name a few. They address problems caused by prostaglandins, chemical compounds in the body that contribute to inflammation in bone joints. The inflammation leads to the recognizable symptoms of pain, such as the swelling and tenderness that sufferers of arthritis are familiar with.
When a patient takes Iconal, the chemicals in the drug block the enzymes that produce prostaglandins and reduce the overall level of those chemical compounds. This results in a decrease in inflammation and the pain symptoms they cause.
For this reason, Iconal is prescribed for the treatment of rheumatoid arthritis, osteoarthritis, and juvenile rheumatoid arthritis. The brand-name formulation of Iconal is Mobic. The drug is administered in capsule, tablet, or liquid forms. The average dose is 5-7.5 mg once a day, with room for adjustment.
