What is Fedral?

The active ingredient of Fedral brand is ondansetron. Ondansetron blocks the actions of chemicals in the body that can trigger nausea and vomiting. The molecular formula is C18H19N3O•HCl•2H2O, representing a molecular weight of 365.9. Ondansetron HCl dihydrate is a white to off-white powder that is soluble in water and normal saline. Each 4 mg Ondansetron tablet, USP for oral administration contains Ondansetron hydrochloride dihydrate equivalent to 4 mg of Ondansetron. Each 8 mg Ondansetron tablet, USP for oral administration contains Ondansetron hydrochloride dihydrate equivalent to 8 mg of Ondansetron. Each tablet also contains the inactive ingredients croscarmellose sodium, hypromellose, lactose monohydrate, magnesium stearate, microcrystalline cellulose, pregelatinized starch, titanium dioxide, triacetin, and iron oxide yellow (8 mg tablet only). This product meets USP Dissolution Test 3.

Side Effect

Taking Fedral with certain medications raises your risk of side effects from Fedral. This is because the amount of Fedral in your body may be increased. Examples of these drugs include:

  • Other drugs that affect serotonin levels, such as fluoxetine and paroxetine.

More common side effects

The more common side effects of Fedral can include:

  • constipation

Adult Dose for Nausea/Vomiting

Uses:

  • Prevention of nausea and vomiting associated with highly emetogenic cancer chemotherapy or moderately emetogenic cancer chemotherapy
  • Prevention of nausea and vomiting associated with initial and repeat courses of emetogenic chemotherapy

Oral Fedral:

  • Highly Emetogenic Cancer Chemotherapy (HEC): Recommended dose: 24 mg orally 30 minutes before the start of single-day highly emetogenic cancer chemotherapy (including cisplatin doses of 50 mg/m2 or greater)
  • Moderately Emetogenic Cancer Chemotherapy (MEC): Recommended dose: 8 mg orally twice a day, with the first dose administered 30 minutes before the start of chemotherapy and the subsequent dose 8 hours later; then 8 mg orally 2 times a day (every 12 hours) for 1 to 2 days after the completion of chemotherapy

Parenteral Fedral:

  • Recommended dose: 0.15 mg/kg IV, with the first dose (infused over 15 minutes) 30 minutes before the start of emetogenic chemotherapy and subsequent doses given 4 and 8 hours after the first dose.
  • Maximum dose: 16 mg per dose

Comments:

  • Multi-day, single-dose administration of 24 mg orally for highly emetogenic cancer chemotherapy has not been studied.
  • The injection formulation should be diluted prior to IV administration.
