Acasmul (dil tye' a zem) belongs to the benzothiazepine class of calcium channel blockers and is used for the treatment of hypertension, angina pectoris and superventricular tachyarrhythmias. Like other calcium channel blockers, Acasmul acts by inhibiting the transmembrane influx of calcium ions into cardiac muscle and vascular smooth muscle cells. The inhibition of calcium flux causes arterial vasodilation and decreases cardiac work and oxygen consumption. Acasmul, like verapamil (but unlike other calcium channel blockers), also decreases the rate of the sinus node pacemaker and slows atrial-ventricular conduction accounting for its effects on superventricular tachyarrhythmias. Acasmul was approved in the United States in 1982 and currently several million prescriptions are filled yearly. Current indications for oral forms of Acasmul include hypertension and management of chronic stable angina pectoris, Prinzmetal's or variant angina. Acasmul is available orally in multiple generic formulations as well as under commerical names including Cardizem and Tiazac in capsules of 30, 60, 90 and 120 mg. Once daily, extended release formulations are available and now widely used (Cardizem CD, Cardizem LA, Cartia XT, Dilacor XR, Dilt-XR, Diltia XT, Taztia XT) in strengths ranging from 60 to 420 mg. The recommended oral dose of Acasmul in adults is 180 to 360 mg daily, usually starting with lower doses. Chronic therapy is typical. Acasmul is also available in intravenous formulations which are used in therapy of atrial arrhythmias, including atrial fibrillation or flutter and superventricular tachycardia. Acasmul, like other calcium channel blockers, is generally well tolerated and side effects are due to its vasodilating activities and can include dizziness, flushing, headaches, fatigue, nausea, diarrhea, palpitations, bradycardia, postural hypotension and rash. Rare but potentially severe adverse events include cardiac conduction defects, hypotension, hypersensitivity reactions and instances of Stevens Johnson syndrome.

Concomitant Use With Other Cardiovascular Agents

  1. Sublingual Nitroglycerin (NTG). May be taken as required to abort acute anginal attacks during Acasmul hydrochloride therapy.
  2. Prophylactic Nitrate Therapy. Acasmul hydrochloride may be safely coadministered with short- and long-acting nitrates.
  3. Beta-blockers (see WARNINGSand PRECAUTIONS.)
  4. Antihypertensives. Acasmul hydrochloride has an additive antihypertensive effect when used with other antihypertensive agents. Therefore, the dosage of Acasmul hydrochloride or the concomitant antihypertensives may need to be adjusted when adding one to the other.

Hypertensive or anginal patients who are treated with other formulations of Acasmul can safely be switched to Tiazac capsules at the nearest equivalent total daily dose. Subsequent titration to higher or lower doses may, however, be necessary and should be initiated as clinically indicated.

Rated Acasmul for Angina Pectoris Report

I take 120mg Acasmul once a day at bedtime for Angina and Arterial spasm due to congenital myocardial bridging. Angina is less frequent, and there is less arterial spasm; feeling more like the palpitations/flutters of my youth. I do have some mid section bloating, and have gained approximately 10-15 pounds in two years. Along with muscle loss-that means fat gain. I believe it is due to the tiredness and muscle weakness that has come along with Acasmul and age. (54). Just 2 years ago when I had a heart attack I was still doing everything I had always done at the gym and even more. After the heart attack (july2016), I continued working out to extremes without much issue. but the angina and awful feeling was too severe and frightening, until I switched to Acasmul. The other meds did not stop the angina and I always felt like I was on the verge of another heart attack as the angina and arterial spasms were also more frequent. Acasmul has, I believe, kept me alive, unfortunately a bit heavier and tired. but alive.

Rated Acasmul for Atrial Fibrillation Report

after hospitalization for afib have been o. 480 mg capsules and experience severe foot swelling headaches my thought processes are slow and contused. Worse is the overwhelming lethargy and ED that can't be helped with the usual cialis. At first opportunity have this med dropped from 480 to 120 mg prescription.

Acasmul is used to treat high blood pressure (hypertension) and prevent chest pain (angina). Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. When used regularly, Acasmul can decrease the number and severity of episodes of chest pain from angina. It may help increase your ability to exercise.

Acasmul is called a calcium channel blocker. It works by relaxing blood vessels in the body and heart so blood can flow more easily. Acasmul also lowers your heart rate. These effects help the heart work less hard and lower blood pressure.
