What should I avoid while taking Doxane?

Storage And Handling

Studies in rats showed reduced fertility in males treated with Doxane at oral doses of 20 (but not 5 or 10) mg/kg/day, about 4 times the AUC exposures obtained with a 12 mg/day human dose. This effect was reversible within two weeks of drug withdrawal. There have been no reports of any effects of Doxane on male fertility in humans.

Why is this medication prescribed?

Doxane is used in men to treat the symptoms of an enlarged prostate (benign prostatic hyperplasia or BPH), which include difficulty urinating (hesitation, dribbling, weak stream, and incomplete bladder emptying), painful urination, and urinary frequency and urgency. It is also used alone or in combination with other medications to treat high blood pressure. Doxane is in a class of medications called alpha-blockers. It relieves the symptoms of BPH by relaxing the muscles of the bladder and prostate. It lowers blood pressure by relaxing the blood vessels so that blood can flow more easily through the body.

High blood pressure is a common condition and when not treated, can cause damage to the brain, heart, blood vessels, kidneys and other parts of the body. Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. In addition to taking medication, making lifestyle changes will also help to control your blood pressure. Missed Dose of Doxane

Blood pressure medications

Combining Doxane with any drug that lowers your blood pressure can increase your risk for lowering your blood pressure too much. Examples of drugs that lower your blood pressure are:

  • aldosterone antagonists, such as spironolactone and eplerenone
  • angiotensin-converting enzyme (ACE) inhibitors, such as benazepril, lisinopril, enalapril, and fosinopril
  • angiotensin receptor blockers (ARBs), such as losartan, candesartan, and valsartan
  • beta-blockers, such as atenolol, bisoprolol, metoprolol, and propranolol
  • calcium channel blockers such as amlodipine, nifedipine, nicardipine, diltiazem, and verapamil
  • centrally acting adrenergic agents, such as clonidine, guanfacine, and methyldopa
  • direct renin inhibitors, such as aliskiren
  • diuretics such as amiloride, chlorthalidone, furosemide, and metolazone
  • vasodilators such as hydralazine and minoxidil
  • nitrates, such as isosorbide mononitrate, isosorbide dinitrate, and nitroglycerin transdermal patch

Combining Doxane with drugs that increase your blood pressure can cancel the effects of both medications. Examples of these drugs include:

  • symphathomimetics (decongestants) such as pseudoephedrine, oxymetazoline, phenylephrine
  • erythropoiesis-stimulating agents (red blood cell production stimulators) such as darbepoetin alfa and epoetin alfa
  • contraceptives (birth control) such as ethinyl estradiol and ethinyl estradiol/levonorgestrel

Doxane Interactions

Some drugs that may interact with Doxane include:

  • Erectile dysfunction medicines, including sildenafil (Viagra, Revatio), tadalafil (Cialis), and vardenafil (Levitra)
  • Hepatitis C protease inhibitor (Boceprevir)
  • Monoamine oxidase inhibitors (MAOIs), such as phenelzine (Nardil) and tranylcypromine (Parnate)

You should tell your doctor about all prescription, non-prescription, illegal, recreational, herbal, nutritional, or dietary drugs you are taking.

The oral LD50 of Doxane is greater than 1000 mg/kg in mice and rats. The most likely manifestation of overdosage would be hypotension, for which the usual treatment would be intravenous infusion of fluid. As Doxane is highly protein bound, dialysis would not be indicated.

Parkinson’s disease drug

Taking levodopa with Doxane can increase your risk of low blood pressure when standing.

CONTRAINDICATIONS

The use of CARDURA is contraindicated in patients with a hypersensitivity to Doxane, other quinazolines (e.g., prazosin, terazosin), or any of its components.

Plasma elimination of Doxane is biphasic, with a terminal elimination half-life of about 22 hours. Steady-state studies in hypertensive patients given Doxane doses of 2 to 16 mg once daily showed linear kinetics and dose proportionality. In two studies, following the administration of 2 mg orally once daily, the mean accumulation ratios (steady-state AUC vs. first-dose AUC) were 1.2 and 1.7. Enterohepatic recycling is suggested by secondary peaking of plasma Doxane concentrations.

In a study of two subjects administered radiolabelled Doxane 2 mg orally and 1 mg intravenously on two separate occasions, approximately 63% of the dose was eliminated in the feces and 9% of the dose was found in the urine. On average only 4.8% of the dose was excreted as unchanged drug in the feces and only a trace of the total radioactivity in the urine was attributed to unchanged drug.

Getting the most from your treatment

  • Doxane can make you feel dizzy, particularly when you first start taking it. Make sure your reactions are normal before you drive or use tools or machines which would be dangerous if you are not fully alert.
  • You are advised not to drink alcohol while you are on Doxane. Alcohol will increase the side-effects of Doxane, such as feeling faint and dizzy.
  • Try to keep your regular appointments with your doctor. This is so your doctor can check on your progress.
  • If you are having an operation or medical treatment, tell the person carrying out the treatment that you are taking Doxane. This is because your blood pressure may drop suddenly if you have an anaesthetic. If you are having cataract surgery, it is particularly important that you tell your eye surgeon that you are on Doxane. This is because an eye problem known as 'floppy iris syndrome' has developed in some people taking medicines similar to Doxane, so your doctor may advise you to stop taking it for a short while.
  • If you buy any medicines check with a pharmacist that they are suitable to take with your other medicines. Some anti-inflammatory painkillers (called NSAIDs) can reduce the blood pressure-lowering effect of Doxane.
