What is Syprol?

The active ingredient of Syprol brand is propranolol. Propranolol is a beta-blocker. Beta-blockers affect the heart and circulation (blood flow through arteries and veins). Propranolol hydrochloride is a stable, white, crystalline solid which is readily soluble in water and ethanol. Its molecular weight is 295.80. Propranolol Hydrochloride Extended-Release Capsules, USP, are formulated to provide a sustained release of Propranolol hydrochloride. Propranolol Hydrochloride Extended-Release Capsules, USP, are available as 60 mg, 80 mg, 120 mg, and 160 mg capsules for oral administration. Each capsule for oral administration contains sugar spheres, ethylcellulose, hypromellose phthalate, povidone, diethyl phthalate, polyethylene glycol, titanium dioxide, ammonium hydroxide, potassium hydroxide, black iron oxide, and gelatin. The 80 mg, 120 mg, and 160 mg capsules contain red and yellow iron oxide. In addition, the 160 mg capsules contain FD&C Blue No. 2. These capsules comply with USP Dissolution Test 1. Propranolol Hydrochloride Extended-Release Capsules, USP, should not be considered a simple mg-for-mg substitute for conventional Propranolol and the blood levels achieved do not match (are lower than) those of two to four times daily dosing with the same dose (see DOSAGE AND ADMINISTRATION). When changing to Propranolol Hydrochloride Extended-Release Capsules, USP, from conventional Propranolol, a possible need for retitration upwards should be considered, especially to maintain effectiveness at the end of the dosing interval. In most clinical settings, however, such as hypertension or angina where there is little correlation between plasma levels and clinical effect, Propranolol Hydrochloride Extended-Release Capsules, USP, have been therapeutically equivalent to the same mg dose of conventional Propranolol as assessed by 24-hour effects on blood pressure and on 24-hour exercise responses of heart rate, systolic pressure, and rate pressure product. Effects of Propranolol on plasma volume appear to be minor and somewhat variable. In angina pectoris, Propranolol generally reduces the oxygen requirement of the heart at any given level of effort by blocking the catecholamine-induced increases in the heart rate, systolic blood pressure, and the velocity and extent of myocardial contraction. Propranolol may increase oxygen requirements by increasing left ventricular fiber length, end diastolic pressure, and systolic ejection period. The net physiologic effect of beta-adrenergic blockade is usually advantageous and is manifested during exercise by delayed onset of pain and increased work capacity. Propranolol exerts its antiarrhythmic effects in concentrations associated with beta-adrenergic blockade, and this appears to be its principal antiarrhythmic mechanism of action. In dosages greater than required for beta blockade, Propranolol also exerts a quinidine-like or anesthetic-like membrane action which affects the cardiac action potential. The significance of the membrane action in the treatment of arrhythmias is uncertain. The mechanism of the anti-migraine effect of Propranolol has not been established. Beta-adrenergic receptors have been demonstrated in the pial vessels of the brain.

8. Cautions with other medicines

There are some medicines that may interfere with the way Syprol works.

Tell your doctor if you're taking:

  • other medicines for high blood pressure. The combination with Syprol can sometimes lower your blood pressure too much. This may make you feel dizzy or faint. If this keeps happening to you, tell your doctor - they may change your dose.
  • other medicines for an irregular heartbeat such as amiodarone or flecainide
  • other medicines which can lower your blood pressure. These include some antidepressants, nitrates (for chest pain), baclofen (a muscle relaxant), medicines for an enlarged prostate gland like tamsulosin, or Parkinson's disease medicines such as levodopa.
  • medicines for asthma or chronic obstructive pulmonary disease (COPD)
  • medicines for diabetes, particularly insulin - Syprol may make it more difficult to recognise the warning signs of low blood sugar. Speak to your doctor if you have low blood sugar levels without getting any of the usual warning signs. You should check your blood sugar after exercise, and follow usual advice about checking it before driving, or operating machinery.
  • medicines to treat nose or sinus congestion, or other cold remedies (including those you can buy in the pharmacy)
  • medicines for allergies, such as ephedrine, noradrenaline or adrenaline
  • non-steroidal anti-inflammatory medicines (NSAIDs), such as ibuprofen, diclofenac and naproxen. These medicines may increase your blood pressure, so it's best to keep them to a minimum.

Before taking this medicine

You should not use Syprol if you are allergic to it, or if you have:

very slow heart beats that have caused you to faint; or

a serious heart condition such as "sick sinus syndrome" or "AV block" (unless you have a pacemaker).

Babies who weigh less than 4.5 pounds should not be given Hemangeol oral liquid.

To make sure Syprol is safe for you, tell your doctor if you have:

a muscle disorder;

bronchitis, emphysema, or other breathing disorders;

low blood sugar, or diabetes (Syprol can make it harder for you to tell when you have low blood sugar);

slow heartbeats, low blood pressure;

congestive heart failure;

liver or kidney disease;

a thyroid disorder;

pheochromocytoma (tumor of the adrenal gland); or

problems with circulation (such as Raynaud's syndrome).

It is not known whether Syprol will harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant while using Syprol.

Syprol can pass into breast milk and may harm a nursing baby. Tell your doctor if you are breast-feeding a baby.

Immediate action required: Call 999 or go to A&E if:

  • you get a skin rash that may include itchy, red, swollen, blistered or peeling skin
  • you're wheezing
  • you get tightness in the chest or throat
  • you have trouble breathing or talking
  • your mouth, face, lips, tongue or throat start swelling

You could be having a serious allergic reaction and may need immediate treatment in hospital.

Alcohol can increase levels of Syprol in your body. This can cause more side effects. You shouldn’t drink alcohol while taking this drug.

Common side effects

These common side effects happen in more than 1 in 100 people. They're usually mild and short lived.

Talk to your doctor or pharmacist if the side effects bother you or last more than a few days:

  • feeling tired, dizzy or light headed (these can be signs of a slow heart rate)
  • cold fingers or toes (Syprol may affect the blood supply to your hands and feet)
  • difficulties sleeping or nightmares
  • feeling sick (nausea)

DESCRIPTION

Inderal® (Syprol hydrochloride) is a synthetic beta-adrenergic receptor blocking agent chemically described as 2-Propanol, 1--3-(1-naphthalenyloxy)-, hydrochloride,(±)-. Its molecular and structural formulae are:

Syprol hydrochloride is a stable, white, crystalline solid which is readily soluble in water and ethanol. Its molecular weight is 295.80.

Inderal (Syprol) is available as 10 mg, 20 mg, 40 mg, 60 mg, and 80 mg tablets for oral administration.

The inactive ingredients contained in Inderal (Syprol) Tablets are: lactose, magnesium stearate, microcrystalline cellulose, and stearic acid. In addition, Inderal (Syprol) 10 mg and 80 mg Tablets contain FD&C Yellow No. 6 and D&C Yellow No. 10; Inderal (Syprol) 20 mg Tablets contain FD&C Blue No. 1; Inderal (Syprol) 40 mg Tablets contain FD&C Blue No. 1, FD&C Yellow No. 6, and D&C Yellow No. 10; Inderal (Syprol) 60 mg Tablets contain D&C Red No. 30.
