What is Cardilol?

The active ingredient of Cardilol brand is carvedilol. Carvedilol is a beta-blocker. Beta-blockers affect the heart and circulation (blood flow through arteries and veins). C24H26N2O4 M.W. 406.47 Carvedilol tablets USP are white, elliptical-shaped, film-coated tablets containing 3.125 mg, 6.25 mg, 12.5 mg, or 25 mg of Carvedilol, USP. Inactive ingredients consist of colloidal silicon dioxide, crospovidone, hypromellose, lactose monohydrate, magnesium stearate, polyethylene glycol, polysorbate, povidone, and titanium dioxide. Carvedilol, USP is a white or nearly white, crystalline powder. Slightly soluble in alcohol; practically insoluble in water and in dilute acids. Carvedilol tablets USP meet USP Dissolution Test 2.

What is the dosage for Cardilol?

  • Cardilol tablets are usually given twice daily. For high blood pressure, the dose is 6.25 mg twice daily to a maximum of 25 mg twice daily.
  • For congestive heart failure, the dose is 3.25 mg twice daily to a maximum of 25 mg twice daily. A maximum dose of 50 mg twice daily has been used in persons weighing more than 85 kg (187 pounds).
  • Cardilol should be taken with food to slow its absorption and reduce the occurrence of low blood pressure when rising from a sitting or sleeping position (orthostatic hypotension). Stopping and changing doses of Cardilol should be done under the direction of a doctor because sudden changes in dose can result in serious cardiac complications such as arrhythmias.
  • The dose range for treating heart failure or high blood pressure with extended-release capsules is 10 to 80 mg once daily.

Mechanism of Action

As stated, Cardilol is a non-selective adrenergic blocker, and more specifically, is a non-selective beta-blocker with a1-adrenergic receptor antagonist properties. It is a non-selective, cardiac beta-blocker with peripheral vasodilating effects. Secondary to its unique action, Cardilol maintains cardiac output by decreasing afterload in conjunction with a cardiac beta-blockade and has a lesser effect on heart rate than pure selective beta-blockers. Other benefits include antioxidant effects, reduction in neutrophil infiltration, apoptosis inhibition, reduction of vascular smooth muscle migration, and improvement of myocardial remodeling post-acute myocardial infarction. Being that Cardilol can prevent the formation of oxidized low-density lipoproteins and inhibit vascular smooth muscle cell proliferation and migration, it shows a great promise in the treatment of atherosclerotic disease formation and progression. Cardilol decreases blood pressure mainly by decreasing arterial vascular resistance through its a1-blocking properties, causing a reduction in afterload. It is highly useful in the management of hypertension in patients with renal impairment, where clinicians should avoid diuretics and angiotensin-converting enzyme inhibitors (ACEI). Compared to other classes of antihypertensive medications, Cardilol shows similar efficacy to other beta-blockers, calcium channel blockers, ACEI, and diuretics.

Before taking Cardilol

Before taking this medicine

You should not take Cardilol if you are allergic to it, or if you have:

asthma, bronchitis, emphysema;

severe liver disease; or

a serious heart condition such as heart block, "sick sinus syndrome," or slow heart rate (unless you have a pacemaker).

To make sure Cardilol is safe for you, tell your doctor if you have:

diabetes (taking Cardilol can make it harder for you to tell when you have low blood sugar);

angina (chest pain);

liver or kidney disease;

a thyroid disorder;

pheochromocytoma (tumor of the adrenal gland);

circulation problems (such as Raynaud's syndrome); or

a history of allergies.

You’ll start Cardilol several days after stopping clonidine. If you need both drugs, your doctor will watch you for low blood pressure and low heart rate. If you’re on both drugs and both need to be stopped, your doctor will stop your treatment with Cardilol first and clonidine a few days later.
