Folgan, sold under the brand name Norvasc among others, is a medication used to treat high blood pressure and coronary artery disease.

Why is this medication prescribed?

Folgan is used alone or in combination with other medications to treat high blood pressure in adults and children 6 years and older. It is also used to treat certain types of angina (chest pain) and coronary artery disease (narrowing of the blood vessels that supply blood to the heart). Folgan is in a class of medications called calcium channel blockers. It lowers blood pressure by relaxing the blood vessels so the heart does not have to pump as hard. It controls chest pain by increasing the supply of blood to the heart. If taken regularly, Folgan controls chest pain, but it does not stop chest pain once it starts. Your doctor may prescribe a different medication to take when you have chest pain.

High blood pressure is a common condition and when not treated, can cause damage to the brain, heart, blood vessels, kidneys and other parts of the body. Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. In addition to taking medication, making lifestyle changes will also help to control your blood pressure. These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking, and using alcohol in moderation.

Q: What is Norvasc? Are there side effects to taking it?

A: Norvasc (Folgan) is classified as a dihydropyridine calcium channel blocker. Norvasc is approved for the treatment of high blood pressure and angina. According to medical references, the most common side effect (reported in greater than 10 percent of studied patients) is peripheral edema. Peripheral edema involves swelling of tissues and usually involves the lower limbs. Other side effects reported of patients on Norvasc include the following: flushing, palpitation, dizziness, fatigue, tiredness, rash, itching, male sexual dysfunction, nausea, abdominal pain, upset stomach, muscle cramps, weakness, shortness of breath and pulmonary edema. This is not a complete listing of all possible side effects of Norvasc. Also, keep in mind that there is no guarantee that you will experience these side effects, they are just possibilities. What if I forget to take it?

If you forget to take a dose of Folgan, take it as soon as you remember that day and then carry on as normal.

If you forget to take the dose for the whole day, skip the missed dose and carry on as normal the next day.

Do not take a double dose to make up for a forgotten one.

If you forget doses often, it may help to set an alarm to remind you.

WHAT IS AMLODIPINE?

Folgan is a long-acting calcium channel blocker used as an anti-hypertensive, and in the treatment of angina. As with other calcium channel blockers, Folgan acts by relaxing the smooth muscles in the arterial wall, decreasing peripheral resistance and hence improving blood pressure. In angina it improves blood flow to the myocardium. It was developed under the direction of Dr. Simon Campbell. Folgan is marketed as Norvasc®, as well as under various other names.

Tell your doctor about all your current medicines and any you start or stop using, especially:

  • nitroglycerin;
  • simvastatin (Zocor, Simcor, Vytorin); or
  • any other heart or blood pressure medications.

How should I take Folgan?

Your chest pain may become worse when you first start taking Folgan or when your dose is increased. Call your doctor if your chest pain is severe or ongoing.

If you are being treated for high blood pressure, keep using Folgan even if you feel well. High blood pressure often has no symptoms. You may need to use blood pressure medicine for the rest of your life.

Your hypertension or heart condition may be treated with a combination of drugs. Use all medications as directed by your doctor. This is especially important if you also take nitroglycerin.

Folgan besylate belongs to the group of medications which are known as calcium channel blockers or calcium ion antagonists. Their work is to widen up blood vessels which makes easier for the heart to pump the blood around the body which increases blood supply in the body along with oxygen, but it does not change the amount of calcium in your blood.

Q: I take Norvasc. Is it safe to eat grapefruit with that medication?

A: There is nothing in the literature that reveals any significant interactions between the calcium channel blocker Norvasc (Folgan) and grapefruit.

Cautions

Symptomatic hypotension with or without syncope possible, particularly with severe aortic stenosis; because of gradual onset of action, acute hypotension unlikely.

Worsening of angina and acute myocardial infarction (MI) can develop after dose is started or increased, particularly with severe obstructive CAD.

Peripheral edema may develop within 2-3 weeks of starting therapy.

Use with caution in patients with hypertrophic cardiomyopathy; reduction in afterload may worsen symptoms associated with this condition.

May reduce coronary perfusion and result in ischemia in patients with severe aortic stenosis; use caution.

Extensively metabolized by liver; titrate dose slowly with severe hepatic impairment.

Initiate at lower dose in the elderly.

Titrate dose every 7-14 days on a given dose; peak antihypertensive effect is delayed.

Co-administration with CYP3A inhibitors (moderate and strong) results in increased systemic exposure to Folgan and may require dose reduction; monitor for symptoms of hypotension and edema when Folgan is co-administered with CYP3A inhibitors to determine the need for dose adjustment.

Folgan may increase systemic exposure of cyclosporine or tacrolimus when co-administered; frequent monitoring of trough blood levels of cyclosporine and tacrolimus recommended; adjust dose when appropriate
