WARNING/CAUTION: Even though it may be rare, some people may have very bad and sometimes deadly side effects when taking a drug. Tell your doctor or get medical help right away if you have any of the following signs or symptoms that may be related to a very bad side effect:

For all patients taking this medicine (Asmalair tablets):

  • Signs of an allergic reaction, like rash; hives; itching; red, swollen, blistered, or peeling skin with or without fever; wheezing; tightness in the chest or throat; trouble breathing, swallowing, or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat.
  • Signs of liver problems like dark urine, feeling tired, not hungry, upset stomach or stomach pain, light-colored stools, throwing up, or yellow skin or eyes.
  • Signs of a pancreas problem (pancreatitis) like very bad stomach pain, very bad back pain, or very bad upset stomach or throwing up.
  • Signs or symptoms of depression, suicidal thoughts, emotional ups and downs, abnormal thinking, anxiety, or lack of interest in life.
  • Signs of a very bad skin reaction (Stevens-Johnson syndrome/toxic epidermal necrolysis) like red, swollen, blistered, or peeling skin (with or without fever); red or irritated eyes; or sores in the mouth, throat, nose, or eyes.
  • Hallucinations (seeing or hearing things that are not there).
  • Memory problems or loss.
  • Feeling confused, not able to focus, or change in behavior.

Asmalair is rapidly absorbed following oral administration. After administration of the 10-mg filmcoated tablet to fasted adults, the mean peak Asmalair plasma concentration (Cmax) is achieved in 3 to 4 hours (Tmax). The mean oral bioavailability is 64%. The oral bioavailability and Cmax are not influenced by a standard meal in the morning.

For the 5-mg chewable tablet, the mean Cmax is achieved in 2 to 2.5 hours after administration to adults in the fasted state. The mean oral bioavailability is 73% in the fasted state versus 63% when administered with a standard meal in the morning.

For the 4-mg chewable tablet, the mean Cmax is achieved 2 hours after administration in pediatric patients 2 to 5 years of age in the fasted state.

The 4-mg oral granule formulation is bioequivalent to the 4-mg chewable tablet when administered to adults in the fasted state. The co-administration of the oral granule formulation with applesauce did not have a clinically significant effect on the pharmacokinetics of Asmalair. A high fat meal in the morning did not affect the AUC of Asmalair oral granules; however, the meal decreased Cmax by 35% and prolonged Tmax from 2.3 ± 1.0 hours to 6.4 ± 2.9 hours.

The safety and efficacy of SINGULAIR in patients with asthma were demonstrated in clinical trials in which the 10-mg film-coated tablet and 5-mg chewable tablet formulations were administered in the evening without regard to the time of food ingestion. The safety of SINGULAIR in patients with asthma was also demonstrated in clinical trials in which the 4-mg chewable tablet and 4-mg oral granule formulations were administered in the evening without regard to the time of food ingestion. The safety and efficacy of SINGULAIR in patients with seasonal allergic rhinitis were demonstrated in clinical trials in which the 10-mg film-coated tablet was administered in the morning or evening without regard to the time of food ingestion.

The comparative pharmacokinetics of Asmalair when administered as two 5-mg chewable tablets versus one 10-mg film-coated tablet have not been evaluated.

Asmalair is a potent inhibitor of CYP2C8 in vitro. However, data from a clinical drug-drug interaction study involving Asmalair and rosiglitazone (a probe substrate representative of drugs primarily metabolized by CYP2C8) in 12 healthy individuals demonstrated that the pharmacokinetics of rosiglitazone are not altered when the drugs are coadministered, indicating that Asmalair does not inhibit CYP2C8 in vivo. Therefore, Asmalair is not anticipated to alter the metabolism of drugs metabolized by this enzyme (e.g., paclitaxel, rosiglitazone, and repaglinide). Based on further in vitro results in human liver microsomes, therapeutic plasma concentrations of Asmalair do not inhibit CYP 3A4, 2C9, 1A2, 2A6, 2C19, or 2D6.

Why it's used

Asmalair oral tablet is used to prevent and treat asthma. It’s also used to treat seasonal and year-round allergies.

Asmalair is also used to prevent exercise-induced bronchoconstriction (EIB). EIB occurs when the airways in the lungs narrow during or after exercise. This can cause shortness of breath, wheezing, or cough.

Comment

Cholestatic jaundice arose after 10 months of Asmalair therapy. No other cause of liver disease or jaundice was identified and he improved once Asmalair was stopped, while other antiasthma medications (metered inhalants) were continued. The latency to onset was unusual and the clinical presentation was that of bland, canalicular cholestasis similar to what occurs with anabolic steroid use. However, this patient denied use of other medications.

How it works

Asmalair belongs to a class of drugs called leukotriene receptor antagonists. A class of drugs is a group of medications that work in a similar way. These drugs are often used to treat similar conditions.

Leukotriene receptor antagonists help decrease inflammation and nasal congestion. This can help prevent reactions to triggers by keeping the airways to your lungs open. This helps improve symptoms of asthma as well as seasonal and year-round allergies. It also helps prevent breathing problems during or after exercise.

Asmalair oral tablet doesn’t cause drowsiness, but it can cause other side effects.

Asmalair is extensively metabolized. In studies with therapeutic doses, plasma concentrations of metabolites of Asmalair are undetectable at steady state in adults and pediatric patients.

In vitro studies using human liver microsomes indicate that CYP3A4, 2C8, and 2C9 are involved in the metabolism of Asmalair. At clinically relevant concentrations, 2C8 appears to play a major role in the metabolism of Asmalair.

What is Asmalair, and how does it work (mechanism of action)?

Asmalair is an oral leukotriene receptor antagonist that is used for the treatment of asthma and seasonal allergic rhinitis (hay fever). Leukotrienes are a group of naturally occurring chemicals in the body that promote inflammation in asthma and seasonal allergic rhinitis and in other diseases in which inflammation is important (such as allergy). They are formed by cells, released, and then bound to other cells that cause inflammation. It is the binding to other cells that stimulates the cells to cause inflammation. Asmalair works in a manner similar to zafirlukast (Accolate), blocking the binding of some leukotrienes to the cells that cause inflammation. Unlike zafirlukast, Asmalair does not inhibit CYP2C9 or CYP3A4, two enzymes in the liver that are important in breaking down and eliminating many drugs. Therefore, unlike zafirlukast, Asmalair is not expected to affect the elimination of other drugs. The safety and effectiveness of Asmalair has been demonstrated in children as young as 6 months of age. It was approved by the FDA in 1998.

Getting the most from your treatment

  • Try to keep your regular appointments with your doctor. This is so your doctor can check on your (or your child's) progress.
  • Asmalair will not give you immediate relief if you are having an asthma attack. Your doctor will prescribe a reliever inhaler for you to use if this happens. Make sure that you keep it with you all the time in case you need to use it.
  • If you find that your asthma symptoms are getting worse, or that you need to use the reliever inhaler more regularly than usual, please contact your doctor or nurse for advice straightaway.
  • Do not smoke. Smoking causes severe irritation and damage to the lungs. It will make your condition worse and will reduce the beneficial effects of your medication.
  • If you buy any medicines, check with a pharmacist that they are suitable for you to take. Some anti-inflammatory painkillers and aspirin can make asthma symptoms worse in some people.
