What is Hidroxizin?

The active ingredient of Hidroxizin brand is hydroxyzine. Hydroxyzine reduces activity in the central nervous system. It also acts as an antihistamine that reduces the effects of natural chemical histamine in the body. Histamine can produce symptoms of itching, or hives on the skin. C 21H 27ClN 2O 2•C 23H 16O 6 M.W. 763.27 Hydroxyzine Pamoate Capsules USP are administered in doses equivalent to 25 mg, 50 mg or 100 mg of Hydroxyzine hydrochloride. In addition, each capsule contains the following inactive ingredients: croscarmellose sodium, magnesium stearate, and pregelatinized corn starch. The 25 mg also contains anhydrous lactose. The 50 mg and 100 mg also contain lactose monohydrate. The capsule shell ingredients for the 25 mg capsule are D&C red no. 28, D&C yellow no. 10, FD&C blue no. 1, FD&C red no. 40, FD&C yellow no. 6, gelatin, and titanium dioxide. The 50 mg capsule shell contains D&C red no. 33, D&C yellow no. 10, FD&C yellow no. 6, gelatin, and titanium dioxide. The 100 mg capsule shell contains D&C red no. 28, D&C red no. 33, D&C yellow no. 10, FD&C blue no. 1, FD&C red no. 40, gelatin, and titanium dioxide. The edible imprinting ink on the 25 mg, 50 mg and 100 mg capsules contains black iron oxide, D&C yellow no. 10 aluminum lake, FD&C blue no. 1 aluminum lake, FD&C blue no. 2 aluminum lake, FD&C red no. 40 aluminum lake, propylene glycol, and shellac glaze.

Indications and Usage

Hidroxizin is sold under the brand names Atarax or Vistaril. It is used as an antihistamine to help reduce the occurrence of sneezing or runny nose. Unlike some antihistamines you can use this drug to treat allergic reactions on the skin, such as hives or contact dermatitis. In some cases, this medication is also used in conjunction with other substances as an anesthesia or to control nausea or vomiting. Hidroxizin is available in tablet and liquid form. Dosage will vary based on the nature of your symptoms and the condition of the patient. Do not take more of your medication than prescribed as this can lead to severe side effects.

If you are taking a tablet form of this drug, take your dose with a full glass of water. Those using the liquid form of this medication should use the dose measuring spoon provided rather than a utensil from their home to ensure that they are getting the proper dose. If you do not have a dose measuring spoon, you can ask your pharmacist for one. If you forget to take a dose, take it as soon as you remember, unless it is close to the time when you would be taking your next dose. Do not double up doses to make up for those you have missed.


Medications that cause drowsiness, including cold or allergy medications, sedatives, narcotic pain relievers, sleeping pills, muscle relaxers, seizure medication, or medication used to control depression or anxiety may increase your reaction to Hidroxizin HCL to an unsafe level. Hidroxizin may make you drowsy or otherwise impair your ability to think and reason at your normal level. Do not drive or perform any tasks that will require your full attention until you know how this drug will affect your system. Drinking alcohol can increase this risk as well as the risk of developing other side effects, and therefore should be avoided when using this drug.

If you suffer from seizures, liver or kidney disease, you may not be able to take this drug. How should I take Hidroxizin (Atarax, Rezine, Vistaril)?

Follow all directions on your prescription label. Your doctor may occasionally change your dose. Do not use this medicine in larger or smaller amounts or for longer than recommended.

Shake the oral suspension (liquid) well just before you measure a dose. Measure liquid medicine with the dosing syringe provided, or with a special dose-measuring spoon or medicine cup. If you do not have a dose-measuring device, ask your pharmacist for one.

Hidroxizin is for short-term use only.

You should not take this medicine for longer than 4 months. Call your doctor if your anxiety symptoms do not improve, or if they get worse.

Store at room temperature away from moisture and heat.

What Is Hidroxizin and How Does It Work?

Hidroxizin is used for the short-term treatment of nervousness and tension that may occur with certain mental/mood disorders (e.g., anxiety, dementia). It is also used to help manage withdrawal symptoms (e.g., anxiety, agitation) in alcoholics.

Other uses include helping to decrease anxiety and nausea before/after surgery or during childbirth or helping certain narcotic pain relievers (e.g., meperidine) work better.

Hidroxizin belongs to a class of medications called antihistamines. It is thought to work by affecting certain natural substances (acetylcholine, serotonin) in your body or by acting directly on certain parts of the brain.

Hidroxizin also blocks a natural substance that your body makes during an allergic reaction (histamine).

Hidroxizin is available under the following different brand names: Vistaril.

Dosage Considerations – Should be Given as Follows:

  • Symptomatic relief of anxiety associated with psychoneurosis and as adjunct in organicdisease states
  • 50-100 mg orally divided every 6 hours or 50-100 mg intramuscularly (IM) divided every 4-6 hours
  • Younger than 6 years old: 50 mg/day orally divided every 6 hours
  • Children 6 years of age and older: 50-100 mg/day orally divided every 6 hours
  • Geriatric: 50-100 mg orally divided every 6 hours or 50-100 mg IM divided every 4-6 hours
  • Continuation of therapy for more than 4 months has not been studied; reassess need for therapy periodically

  • Management of pruritus due to chronichives (urticaria), contact dermatoses, and histamine-mediated pruritus
  • 25 mg oral/intramuscular (IM) divided every 6-8 hours
  • Younger than 6 years old: 50 mg/day orally divided every 6 hours
  • Children 6 years of age and older: 50-100 mg/day orally divided every 6 hours
  • Geriatric: 25 mg oral/IM every 6-8 hours; increased to 25 mg every 6-8 hours

  • Adult: 50-100 mg orally or 25-100 mg intramuscularly (IM)
  • Pediatric: 0.6 mg/kg orally, or 0.5-1.1 mg/kg IM
  • If treatment is initiated IM, subsequent doses may be administered orally

  • Greater than 50 mL/min: Dose adjustment not necessary
  • 50 mL/min or more: Administer 50% normal dose

  • Change dosing interval to every 24 hours in patients with biliarycirrhosis

  • Advanced age associated with reduced drug clearance and with greater risk of confusion, dry mouth, constipation, and other anticholinergic effects and toxicity; start at low end of dosage range, and observe closely

Hidroxizin HCL is used to decrease activity in the central nervous system, and can also be used as an antihistamine to reduce allergic reactions and similar symptoms. Given the wide variety of uses, it is difficult to determine how this medication should be administered, so you will need to follow the instructions set by your doctor. Do not make any changes to your prescription before speaking to your doctor and receiving permission to do so. You will also need to talk with your doctor about how to alter other prescriptions you may be using as these can cause a negative reaction in the body if paired incorrectly.

What Are Side Effects Associated with Using Hidroxizin?

Side effects associated with use of Hidroxizin, include the following:

  • Dry mouth
  • Drowsiness (usually transitory and may disappear in a few days of continued therapy or upon reduction of the dose)
  • Involuntarymotor activity (tremor, convulsions) usually with doses considerably higher than those recommended
  • Clinically significant respiratory depression has not been reported at recommended doses

Hidroxizin HCL Side Effects

Some side effects of this drug include dry mouth, drowsiness and shakiness. These side effects are considered bothersome, but not necessarily dangerous. It is not known what percentage of the population suffers from these side effects. In most cases, these side effects become less severe as you continue to use your medication. If you begin to suffer from these effects, or your side effects begin to get worse, contact your doctor to determine if a change in your prescription is necessary.

If you begin to suffer from tremors, confusion, seizures, or restless muscle movements in the neck, mouth, or face, you may be suffering from a severe reaction to your medication. Contact emergency medical services immediately. Have as much information as possible available regarding how much of your prescription you have taken and how recently.

Should you begin to experience nausea, vomiting, or feeling as though you may pass out after taking your prescription, you may be suffering from an overdose. Contact emergency medical services as quickly as possible to get the necessary assistance to treat this condition.

  • Capsules, tablets, and liquids should be stored at room temperature 15 C to 30 C (59 F to 86 F).
  • Liquid should not be frozen and should be shaken well prior to each use.
  • Injectable Hidroxizin should be stored below 30 C (86 F).

Q: I take Hidroxizin 25 mg twice a day. It makes me groggy. Is there a dosage that's lower but that gets the job done?

A: Hidroxizin (Vistaril, Atarax) is available in lower dosage forms. Dizziness is listed as a common reaction or side effect to taking this medication. However, side effects can be dose related but they are patient-specific and may just be related to the drug itself. You should check with your physician to see if he feels that your side effects are more likely from the dose or the drug. If it is the drug, then possibly you could be changed to a different anti-anxiety medication altogether.

  • The body releases histamine during several types of allergic reactions and, to a lesser extent, during some viral infections, such as the common cold. When histamine binds to its receptors on cells, it causes the cells to release chemical messengers that lead to sneezing, itching, and increased mucus production. Antihistamines, like histamine, bind to the histamine receptors. When they bind to the receptors, however, they do not stimulate the cells to release chemical messengers. In addition, they prevent histamine from binding and stimulating the cells. Hidroxizin itself has no histamine-like activity. After ingestion, it is converted to its active form. The active form of Hidroxizin is a drug called cetirizine (Zyrtec). Although, both Hidroxizin and cetirizine act as antihistamines, Hidroxizin causes more sedation than cetirizine.


Hidroxizin (hye drox' i zeen) is a first generation antihistamine that is used mostly to treat itching and nausea. Because of its sedating effects, Hidroxizin is also used for anxiety, tension and as a mild sleeping aid. Hidroxizin belongs to the piperazine class of antihistamines (with cyclizine and meclizine) which are more commonly used for itching, nausea and anxiety than for their effects on symptoms of allergic rhinitis or coryza. Hidroxizin was approved for use in the United States in 1957 and continues to be widely used. It is available as tablets or capsules of 10, 25, 50 and 100 mg in multiple generic forms and under the trade names Atarax and Vistaril. Hidroxizin is also available as an oral suspension or syrup and as a liquid for injection. Most formulations are available without prescription. The recommended adult oral dose ranges from 25 to 100 mg three to four times daily. Doses used for itching are generally lower than those for anxiety and tension. Common side effects include sedation, impairment of motor function, confusion, dizziness, blurred vision, dry mouth and throat, palpitations, tachycardia, abdominal distress, constipation and headache. Antihistamines can worsen urinary retention and glaucoma.

What should I avoid while taking Hidroxizin?

This medicine may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert.

Drinking alcohol with this medicine can cause side effects.

Hidroxizin side effects

Hidroxizin may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:

  • dry mouth
  • constipation (especially in older adults)
  • confusion (especially in older adults)
  • dizziness
  • headache

Some side effects can be serious. Stop using Hidroxizin and call your doctor at once if you experience any of the following symptoms:

  • fast or pounding heartbeats;
  • headache with chest pain;
  • severe dizziness, fainting;
  • unintentional trembling or shaking movements;
  • a seizure (convulsions).

If you experience any of the following symptoms of a serious skin condition; stop taking Hidroxizin and call your doctor immediately:

  • rash
  • pus-filled, blister-like sores (lesions), areas of swelling and redness on the skin, and fever

Hidroxizin may cause other side effects. Call your doctor if you have any unusual problems while you are taking this medication.
