What is doxycycline? What is Novatrex?

Doxycycline is a tetracycline antibiotic used to treat many different types of infections, including respiratory tract infections due to Hemophilus influenzae, Streptococcus pneumoniae, or Mycoplasma pneumoniae. It is used also in the treatment of syphilis, acne, periodontal disease, non-gonococcal urethritis (due to Ureaplasma), Rocky Mountain spotted fever, typhus, chancroid, cholera, brucellosis, and anthrax.

Novatrex is a macrolide antibiotic related to erythromycin and clarithromycin (Biaxin) used to treat bacterial infections such as otitis media (infection of the middle ear), tonsillitis, laryngitis, bronchitis, pneumonia, sinusitis, uncomplicated skin and skin structure infections, Mycobacterium avium complex, acute bacterial flare ups of chronic obstructive pulmonary disease (COPD), acute pelvic inflammatory disease, and several sexually transmitted infectious diseases (STDs) such as non-gonococcal urethritis and cervicitis.

How should I take Novatrex?

Follow all directions on your prescription label and read all medication guides or instruction sheets. Use the medicine exactly as directed.

Novatrex oral is taken by mouth. Novatrex injection is given as an infusion into a vein, usually for 2 days before you switch to Novatrex oral. A healthcare provider will give you this injection.

You may take Novatrex oral with or without food.

Shake the oral suspension (liquid) before you measure a dose. Use the dosing syringe provided, or use a medicine dose-measuring device (not a kitchen spoon).

Use this medicine for the full prescribed length of time, even if your symptoms quickly improve. Skipping doses can increase your risk of infection that is resistant to medication. Novatrex will not treat a viral infection such as the flu or a common cold.

Store at room temperature away from moisture and heat. Throw away any unused liquid medicine after 10 days.

Never take more than the prescribed dose. If you suspect that you or someone else might have taken an overdose of this medicine, go to the accident and emergency department of your local hospital. Take the container with you, even if it is empty.

This medicine is for you. Never give it to other people even if their condition appears to be the same as yours.

DOSAGE AND ADMINISTRATION

ZITHROMAX for oral suspension (single dose 1 g packet) can be taken with or without food after constitution. However, increased tolerability has been observed when tablets are taken with food.

Zithromax for oral suspension (single dose 1 g packet) is not for pediatric use. For pediatric suspension see the prescribing information for ZITHROMAX (Novatrex for oral suspension) 100 mg/5 mL and 200 mg/5 mL bottles.

Directions for administration of ZITHROMAX for oral suspension in the single dose packet (1 g): The entire contents of the packet should be mixed thoroughly with two ounces (approximately 60 mL) of water. Drink the entire contents immediately; add an additional two ounces of water, mix, and drink to ensure complete consumption of dosage. The single dose packet should not be used to administer doses other than 1000 mg of Novatrex.

Prophylaxis of sexually transmitted diseases (STDs) such as gonorrhea after sexual assault per CDC guidelines includes the following 3-drug regimen:

  • Ceftriaxone 250 mg intramuscularly (IM) once PLUS
  • Novatrex 1 g orally once PLUS
  • Metronidazole or tinidazole 2 g orally once
  • If alcohol has been recently ingested or emergency contraception is provided, metronidazole or tinidazole can be taken by the victim at home rather than as directly observed therapy to avoid drug interactions

Common side effects

These common side effects of Novatrex happen in more than 1 in 100 people. Keep taking the medicine, but talk to your doctor or pharmacist if these side effects bother you or don't go away:

  • feeling sick (nausea)
  • diarrhoea or being sick (vomiting)
  • losing your appetite
  • headaches
  • feeling dizzy or tired
  • changes to your sense of taste

Common Side Effects of Novatrex

Many people experience fewer side effects when taking Novatrex compared with other antibiotics in the macrolide class, such as erythromycin (Erythrocin).

However, you may still experience side effects, especially if your doctor prescribed a high dose of Novatrex to be taken just one time. Tell your doctor if you experience any of the following:

  • Nausea
  • Vomiting
  • Diarrhea
  • Gas
  • Loose stools
  • Stomach discomfort

Some people may also experience cramps and yeast or vaginal infection.

ZITHROMAX should be taken at a daily dose of 600 mg, in combination with ethambutol at the recommended daily dose of 15 mg/kg. Other antimycobacterial drugs that have shown in vitro activity against MAC may be added to the regimen of Novatrex plus ethambutol at the discretion of the physician or health care provider.

Novatrex

Novatrex exhibits significant intracellular penetration and concentrates within phagocytes and fibroblasts, thus the levels in tissues are much greater than in plasma. This drug has a long half-life in children (32 to 64 hours). Through selective uptake by fibroblasts and phagocytic cells, tissue concentrations may be 100 to 1000 times that of blood. 7 Novatrex is available in 250-, 500-, and 600-mg oral tablets and as a 100-mg/5 mL and 200-mg/5 mL powder for suspension. IV Novatrex is sometimes combined with an IV β-lactam when atypical pathogens are suspected for inpatients with community-acquired pneumonia. 8

The most common adverse effects in pediatric patients receiving Novatrex are GI effects which are dose-related. Novatrex is not appreciably metabolized.

Contraindications

History of cholestatic jaundice or hepatic impairment associated with prior Novatrex use
